Liver transplantation remains the definitive treatment for end-stage liver disease and selected cases of acute liver failure and liver cancer. Advances in surgical techniques, donor selection, and perioperative management have significantly improved survival outcomes. Post-transplant care focuses on immunosuppression management, prevention of rejection, infection control, and long-term monitoring for metabolic and cardiovascular complications. Recurrence of underlying liver disease and de novo malignancies pose ongoing challenges. Emerging strategies include individualized immunosuppression, improved organ preservation, and optimization of long-term quality of life. This topic addresses advances in transplantation practices, post-transplant monitoring, and strategies to improve graft and patient survival.
